Scheduler- Milford, Per Diem

Position: Scheduler- Milford, Per Diem (As Needed)
Yale New Haven Health -  -  - Responsibilities:
Performs patient-centered scheduling duties, utilizing the Epic scheduling system, and assigns cases to appropriate interdisciplinary team members;
Ensures visits are scheduled for patients appropriately, within agency standards for initiation of services;
Runs reports for clinicians and clinical managers to ensure completion of clinical documentation;
Responsible for tracking and ordering patient supplies under the direction of the interdisciplinary team;
Checks weekly timesheets for correctness and interacts with clinical staff to fix errors
