Physical Therapist (PT) – Full-Time
West River Civita Care Center
Milford, CT
$45–$50/hour
West River Civita Care Center is seeking a motivated Physical Therapist (PT) to join our rehabilitation team. This full-time opportunity is ideal for a clinician who enjoys helping residents regain mobility, improve strength, and return to greater independence in a supportive skilled nursing and post-acute care environment.
What You'll Do- Perform physical therapy evaluations and develop individualized treatment plans
- Provide skilled therapy focused on mobility, strength, balance, gait, transfers, and endurance
- Monitor resident progress and adjust treatment plans based on clinical needs
- Educate residents and caregivers on exercises, mobility techniques, and safety strategies
- Collaborate with PTAs, OTs, COTAs, SLPs, nursing, and other members of the interdisciplinary team
- Complete accurate and timely clinical documentation
- Collaborative and supportive rehabilitation department
- Diverse short-term rehabilitation and long-term care caseload
- Resident-centered approach focused on meaningful functional outcomes

- Graduate of an accredited Physical Therapy program
