Sleep Medicine Advanced Practitioner

Hartford Healthcare seeks a Sleep Medicine Advanced Practitioner to join our Healthcare & Medical Services team. In this role, you will evaluate and manage patients with sleep disorders, interpret sleep study results, and develop evidence-based treatment plans. You'll collaborate closely with physicians and an interdisciplinary team to deliver high-quality, compassionate care, while educating patients and families on sleep health. Responsibilities

• Conduct comprehensive evaluations of patients with suspected sleep disorders
• Interpret polysomnography and other sleep study results under physician collaboration
• Develop and adjust evidence-based treatment plans for sleep conditions such as sleep apnea, insomnia, and restless leg syndrome
• Prescribe and manage medications and therapies within scope of practice
• Provide patient and family education on sleep hygiene, treatment options, and equipment use (e.g., CPAP/Bi
• PAP)
• Collaborate with physicians, technologists, and interdisciplinary team members to coordinate care
• Document visits, assessments, and treatment plans in the electronic health record
• Participate in quality improvement initiatives and clinical protocols within sleep medicine
• Support continuity of care through follow-up visits, telehealth, and care coordination
• Engage in ongoing professional development and sleep medicine training
