Bridges Healthcare, Inc., has an immediate opening for a Prevention Community Engagement Specialist. Salary: $24.07 - $28.07 per hour, commensurate of experience.

Purpose and

Position Overview The Prevention Program /Case Manager provides prevention, case management, outreach, and program support services to youth, young adults, and families. This position works collaboratively with schools, community partners, coalition members, and service providers to implement prevention initiatives, provide education and support services, coordinate community resources, and assist with program operations. The role requires a combination of direct client services, community engagement, program coordination, data collection, and administrative support.

Duties and Responsibilities Provide office-based, school-based, or community-based case management and support services to youth, young adults, and families.

Conduct outreach and engagement activities with community stakeholders, schools, coalition members, and service providers.

Facilitate prevention education, health promotion, parenting support, and other evidence-based prevention activities.

Coordinate and participate in community prevention initiatives, meetings, trainings, and special events.

Maintain accurate client and program records, collect data, and assist with reporting requirements.

Serve as a liaison between clients, families, schools, community organizations, and other service providers.

Collaborate with multidisciplinary teams to identify needs, coordinate resources, and make appropriate referrals.

Assist with the promotion and marketing of prevention programs and services.

Support program operations through administrative tasks, scheduling, documentation, and project management activities.

Participate in committee work, coalition activities, and community partnerships that advance prevention efforts.

Perform other agency or department-related duties and special projects as assigned.

Qualifications and Competencies Bachelor's degree required; degree in Human Services, Social Work, Public Health, Health Education, Counseling, Psychology, or related field preferred. Master's degree may be preferred for certain program assignments.

Experience working with youth, young adults, families, schools, or community-based programs preferred.

Knowledge of prevention strategies, behavioral health, substance use prevention, restorative practices, and community resources.

Ability to provide case management, coordinate care, and facilitate referrals.

Strong communication, organizational, documentation, and interpersonal skills.

Ability to work collaboratively with diverse populations and community partners.

Computer literacy and proficiency with documentation and data management systems.

Ability to prioritize multiple responsibilities and work independently.

Flexible schedule as needed to support program activities and community events.
