Pay or shift range: $35 USD to $38 USD per hour. The range is the budgeted amount for this position; final offers are based on skill set, experience, location, qualifications, and other job-related factors.

The Home Health LPN is an integral part of our inter‑disciplinary care team that provides care to patients in their homes. We pay hourly, offer a 32‑40 hour work week, and have a flexible AM start time to support work/life balance. We are currently hiring full‑time LPNs in the Milford area.

• Make home visits to patients living in designated territories.
• Perform assigned duties including medication administration, wound care, IV therapy, treatments, and procedures.
• Teach and educate patients and/or caregivers on disease process, medications, diet, wound care, and other treatments as ordered by the physician.
• Communicate regularly with the RN Case Manager and the patient inter‑disciplinary care team to ensure the most appropriate care is being provided.
• Complete and submit accurate, timely clinical documentation pertaining to patient interventions and care provided.

• Licensed Practical Nurse with a current license in the state of Massachusetts.
• Minimum of two years of experience delivering nursing care.
• Minimum of one year of Home Health experience preferred.
