Licensed Vocational Nurse (LVN/LPN)

Seeking a licensed vocational nurse (LVN/LPN) to provide quality patient care in long-term care or skilled nursing facility settings. The ideal candidate will adhere to healthcare standards and deliver compassionate, efficient support to patients.

Responsibilities:

• Administer prescribed medications and treatments accurately according to physician orders.
• Monitor patient vital signs and assess changes in health status.
• Document patient care activities and observations thoroughly in medical records.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Ensure compliance with facility policies, state, and federal regulations.
• Maintain a safe, clean, and welcoming environment for patients.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in LTC or SNF settings preferred.
• Strong knowledge of medication administration and patient safety protocols.
• Ability to follow detailed instructions and document care accurately.
• Excellent communication and interpersonal skills.
• Must bring a valid state or federal  wear a photo badge at all times.
