Skilled Nursing Facility (long-term care). This is a travel - temporary assignment through Healthforce Healthcare LLC. Local candidates only; no 12-hour shifts available.

• Administer prescribed medications and treatments per physician orders and facility policy.
• Monitor residents’ conditions, document vital signs, and report changes to the supervising RN.
• Assist residents with activities of daily living, promoting dignity, safety, and mobility.
• Perform routine wound care, dressing changes, and other standard nursing tasks as directed.
• Document care provided and resident responses in the resident chart or EMR; communicate with the care team to support care planning.

• Active Licensed Practical Nurse license in New Hampshire
• Current Basic Life Support (BLS) certification
• Minimum 1 year of licensed practical nurse experience in a long-term care or skilled nursing setting
• Graduation from an approved LPN program
• Ability to work evenings and perform duties on-site in Milford, NH for a 7-week assignment
