LVN ​/ LPN – LTC​/SNF

Job Summary:

Provide skilled Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) care in a long-term care or skilled nursing facility setting. Ensure resident needs are met with professionalism and adherence to healthcare standards.

Responsibilities:

• Administer medications and treatments to residents as prescribed by physicians.
• Monitor and assess residents' health status, reporting changes to the healthcare team.
• Document patient care activities accurately and timely.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Maintain a safe, clean, and comfortable environment for residents.
• Adhere to all facility policies, including COVID-19 vaccination requirements and identification protocols.

Qualifications:

• Valid state LVN or LPN license or license eligibility.
• Recent experience in LTC or SNF settings preferred.
• Ability to follow detailed care plans and procedures consistently.
• Strong communication skills and attention to detail.
• Must present a valid  comply with COVID-19 vaccination or exemption requirements.
