Licensed Practical Nurse (LPN) - The Elms Center

The Elms Center, a reputable skilled nursing facility in Milford, New Hampshire, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our exceptional team! As an LPN at The Elms Center, you will have the opportunity to provide high-quality care to our residents while working in a supportive and dynamic environment.

Part-time 7AM - 3PM | Part-time 11PM - 7AM

$36.00 - $42.00, based on experience + applicable shift differentials

Full Time $5,000 | Part Time $2,500

• Leading and directing daily nursing activities in alignment with facility policies
• Participating in resident admissions, transfers, and discharges
• Assessing resident conditions and promptly notifying physicians and families
• Transcribing and carrying out physician orders accurately
• Preparing and administering medications and monitoring for adverse reactions
• Performing wound treatments and documenting healing progress
• Coordinating follow-up appointments and transportation
• Providing advanced nursing procedures such as IV administration, suctioning, tube feedings, colostomy care, and more
• Conducting rounds to ensure resident needs are met and staff duties are completed
• Collaborating with interdisciplinary team members for holistic care
• Participating in care plan development, updates, and resident/family meetings
• Maintaining adequate supplies, medications, and equipment
• Supporting performance evaluations, coaching, and discipline as directed
• Assisting with QAPI, infection control, and survey readiness
• Responding to incidents and participating in investigations

Your leadership helps create a safe, supportive, and respectful environment for residents and staff alike.

• Current, unrestricted LPN license in New Hampshire or a valid multistate compact nursing license with authority to practice in New Hampshire
• CPR certification
• Long-term care experience preferred
• Ability to meet physical demands (standing, bending, lifting 35 lbs, etc.)
• Willingness to work occasional weekends or holidays

Eligible full-time employees may elect Modified Compensation calculated at 10-20% of the base rate in lieu of select benefits.
