Overview

SEM Haven is a resident‑centered organization that values choice, dignity, and independence for older adults. The Care Partner works cooperatively and respectfully with residents, coworkers, families, visitors, and volunteers to create a positive, pleasant environment in the home.

The role includes providing for residents’ physical, social, and environmental needs in accordance with facility policies, quality‑improvement guidelines, and all applicable local, state, and federal regulations. The Care Partner promotes residents’ opportunities to engage in normal life pursuits and helps maintain or improve functioning levels while participating in resident‑centered care and team‑based management. The Care Partner assists with daily living activities in assigned household(s), supporting each resident’s individual goals.

• Assist with food preparation and serving.
• Pick up clutter, remove trash, and clean spills.
• Transport residents as needed.
• Attend household meetings, learning circles, and fulfill simple resident requests.
• Support residents’ social and environmental needs in line with quality standards.

Success in this role requires the following skills:

• Ability to implement and evaluate resident care programs.
• Patience, active listening, and strong verbal/non‑verbal communication.
• Interpersonal skills and respect for all people—residents, coworkers, family, healthcare providers, volunteers, and visitors.
• Respect for possessions—maintain clean, orderly, presentable living and work areas.
• Flexibility to modify job duties for the overall benefit of the home and residents.

Physical demands include:

• Mobility: move freely around the facility.
• Lifting: regularly up to 50 lbs.
• Climbing/balancing, stooping/bending, pushing: regularly (up to 25 lbs).
• Standing, sitting, walking: regular, prolonged.
• Reaching: regularly.
• Speaking and reading/writing: must be able to speak, read, and write English clearly.
• Vision and hearing: sufficient to meet job requirements; prosthetics may be used if needed.

The ADA guidelines provide reasonable accommodations for staff who cannot perform these essential functions.

• Certified Nursing Assistant in the state of Ohio, registered or eligible for testing.
• Training in food sanitation, housekeeping procedures, restorative nursing, and resident activities.
• Free from illegal drug use and from the negative effects of drugs and alcohol in the workplace.
• Ability to read, write, and speak English fluently.
