Responsibilities
  • Serve as the point of contact for referrals and obtain necessary information to initiate and facilitate the patient's transfer into RWJBH Healthcare hospitals under administration.
  • Proficient in all Transfer Center workflows and protocols.
  • Work directly with staff at each client facility to ensure timely access, appropriate bed assignment, and accurate patient information documentation to assist with an appropriate destination plan of care at the receiving facility; receive, document, and communicate time-sensitive clinical data from multiple sources.
  • Demonstrate the ability to triage and prioritize patient information.
  • Assign caseload to clinical and support staff and serve as a liaison between behavioral health staff and providers for complex comorbidities.
  • Coordinate transport and pick-up times.
  • Proficient in Transfer Center database platform, EMR, telephony systems, and Office systems to support operations, ensuring accurate, timely data entry and PHI protection.
  • Monitor compliance with regulatory components (ethics, quality initiatives, HIPAA) and implement process changes to ensure compliance.
  • Support system, facility and department goals and values; align with RWJBH hospital growth strategies.
  • Demonstrate organizational and time-management skills; analyze and solve problems and collaborate to make timely decisions.
  • Adhere to safety, infection control, and company policies; ensure quality and compliance with procedures.
Qualifications

Required:

  • Graduation from an approved school of nursing
  • New Jersey Professional Registered Nurse License
  • At least 2 years in a Behavioral Health patient care setting
  • Experience typically gained within 2-3 years in a patient-facing Behavioral Health setting; strong educational background
  • Excellent verbal and written communication, organizational, and critical thinking skills

Preferred:

  • Bachelor's degree in nursing
  • Two years of supervisory experience
  • Ability to type at least 40 wpm
  • Certifications and Licenses

    Required:

    New Jersey Professional Registered Nurse License

Other Duties
  • Assist with non-patient-related activities.
  • Assist in putting away supplies and containment of costs via efficient use of supplies and equipment.
  • Report malfunctioning or missing equipment; maintain a safe and clean environment.
  • Use good communication skills; show initiative; provide courteous service to all customers.
  • Precept new employees and assist with telephone communications.
