Job Description

Transitional Assistance Support Staff-Worcester - ( 260004YV )

Description

Mass Ability prides itself on being a state agency that helps support eligible individuals in leading their most independent lives. As part of the Moving Forward Program (MFP) division of Mass Ability, eligible Participants are supported in finding housing and transitioning from Skilled Nursing Facilities and other restrictive settings to less restrictive settings in the community, where they can access a variety of supports through eligible programs such as the Home and Community-Based Service Waivers and the MFP-Demonstration.

The Transitional Assistance Program is a cornerstone of this work in that we can work with contracted providers to gather necessary housing documents, find housing, purchase allowable, quality, and necessary household items, and ultimately assist participants in transitioning to the community, where they can take advantage of other supports to remain as independent as they choose to their best ability.

Mass Ability seeks an outgoing, process-orientedteam player to join the Transitional Assistance Support Staff team. To be successful in this role, you must be detail-oriented, collaborative, and a stellar communicator. You enjoy teaching others and are motivated to make a difference in the lives of others.

In the capacity of a Transitional Assistance Support Staff member
, you are responsible for the oversight, management, training, and ongoing facilitation of the program. This includes oversight of billing and payments for all Transitional Assistance (TA) services and the Housing Search Entity (HSE). Both programs will require the Transitional Support Staff member to work efficiently and process payments on time, ensuring proper and timely documentation of service provisions consistent with TA plans.

The TA Support Staff members will also provide training to Mass Ability, Aging Services Access points (ASAP), Senior Care Options (SCO), and DDS staff to ensure all agencies understand the purpose and parameters of TA and HSE. In addition, you will deliver ongoing training to TA and HSE providers to ensure that they know their role in the TA system and the requirements of the system, with an emphasis on billing.

In addition, you will assist the MFP Business Improvement Partner (BIP) with recruiting and onboarding new HSE providers.

At Mass Ability, we are driven by a fundamental belief that we are all different but connected – individuals who share a common purpose and who bring our own talents, ingenuity, and perspective to achieve our vision. That belief puts diversity, equity, and inclusion at the center of everything we do. We are keenly aware of the barriers that many of us face as we strive to live life on our own terms.

That's why, as a community, we are committed to always being part of the solution. This work is essential to who we are as an organization. Together, we are making a better state possible for everyone. As a team member, you'll join an agency that values a learning and participatory performance culture with an emphasis on high-performance teams, individual contribution/impact, and engagement in activities to promote and support programs, divisions, and the overall agency vision and mission.
