Utah Healthcare Corps

AUCH Cancer Screening (CS) Patient Navigators (PNs) use their life experiences and knowledge of their communities to reduce barriers to healthcare and build capacity through resource connection, health education, and health system navigation. With over 60 years of evidence-backed interventions across multiple health domains, PNs play an integral role in addressing healthcare access and providing a holistic healthcare approach.

UHC, an Ameri Corps program of the Association for Utah Community Health (AUCH), currently supports more than 20 members serving in over 10 sites throughout the state of Utah. UHC members make a difference in the community by providing vital health education and connections to health and social resources to improve patient physical and mental health.

• Helping health center patients develop their capacity and access to resources, including colorectal cancer screenings, food, housing, quality care and health information
• Helping to build understanding of preventive screenings to support healthier behaviors and lifestyle choices
• Supporting health centers in improving care access by coordinating with gastroenterology centers and local hospitals.
• Providing informal counseling using Motivational Interviewing (MI), support and follow-up
• Improving access to health care services and information through resources, educational programs, activities and referrals.
• Other duties as assigned by clinic or supervisors must be in accordance with Ameri Corps terms and conditions
