Overview

STGi is currently seeking a Minimum Data Set Director (MDS) to provide services at the Georgia War Veterans Home. The primary purpose of this position is to oversee the Minimum Data Set (MDS) process, ensure compliance with state and federal regulations, and promote proper clinical reimbursement for services rendered. Assess charts and communicate with the health care team to create applicable health care plans for residents.

Functions
  • Completing and assuring the accuracy of the MDS process for all residents.
  • Overseeing the collection and analysis of patient data to ensure accurate assessments and documentation.
  • Working with nurses, doctors, and other healthcare professionals to develop individualized care plans for residents.
  • Adhering to all state and federal documentation requirements and participating in regular audits and inspections.
  • Developing and implementing appropriate plans of action to correct identified quality deficiencies.
  • Completing necessary forms, reports, etc., and submitting to the Administrator as required.
  • Hosting and attending interdisciplinary meetings to discuss residents' needs and care plans.
  • Explaining the level of care provided and addressing any concerns or questions.
  • Checking the facility's census daily and completing Discharge and Re-entry Tracking forms as needed.
  • Tracking and scheduling required resident assessments per state and federal requirements.
  • Keeping care plan team members informed of resident assessment scheduling changes and status updates.
  • Reviewing the daily 24-Hour Reports and telephone orders on each unit for significant changes in resident conditions that might alter assessments and care plans.
  • Participating in facility surveys as necessary or as directed.
  • Completing resident assessments to capture the highest possible PDPM score.
  • Educating and training staff members about the assessment process.
  • Ability to work as an RN, rounding with physicians as needed for optimum staffing.
  • Completing all MDS assessments and Care Area Assessment Summaries (CAAs).
  • Participating in the facility quality of care committees and meetings.
  • Completing or ensuring that the Unit Nurses complete special care quarterly assessments.
Required Skills

REQUIRED

EXPERIENCE AND SKILLS:

  • Sufficient prior experience in clinical health care including direct experience in MDS.
  • Ability to make independent decisions when circumstances warrant such action in a timely fashion.
  • Supportive team member, contribute to and lead through teamwork and team concepts.
  • Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the public.
  • Trustworthy and reliable with high ethical standards; leadership and supervisory ability and willingness to work harmoniously with others.
  • Demonstrates respect for the nursing profession by maintaining accountability, integrity and quality patient care.
  • Patience, tact, a cheerful disposition and enthusiasm; ability to handle difficult residents and staff.
  • Willingness to seek out new methods and principles and incorporate them into practices.
Required Experience

REQUIRED

EDUCATION:

  • Must have a minimum of a two-year degree.
  • Must be a Registered Nurse.
Working Conditions /

Physical Requirements
  • Works in office areas as well as throughout the facility and its premises. Subject to frequent interruptions.
  • Involved with residents, family members, personnel, and visitors under all conditions/circumstances.
  • Works beyond normal working hours and on weekends and holidays when necessary.
