STGi is seeking a Minimum Data Set Director (MDS) to provide services at the Georgia War Veterans Home. The primary purpose of this position is to oversee the MDS process, ensure compliance with state and federal regulations, and promote proper clinical reimbursement. This includes assessment of charts and collaboration with the health care team to develop appropriate health care plans for current and incoming residents.

Essential

Functions
  • Complete and assure the accuracy of the MDS process for all residents.
  • Oversee the collection and analysis of patient data to ensure accurate assessments and documentation.
  • Collaborate with nurses, doctors, and other health care professionals to develop individualized care plans for residents.
  • Adhere to all state and federal documentation requirements and participate in regular audits and inspections.
  • Develop and implement plans of action to correct identified quality deficiencies.
  • Complete necessary forms and reports, and submit to the Administrator as required.
  • Host and attend interdisciplinary meetings to discuss residents' needs and care plans.
  • Explain the level of care provided and address any concerns or questions.
  • Check the facility census daily and complete Discharge and Re-entry Tracking forms as needed.
  • Track and schedule required resident assessments per state and federal requirements.
  • Keep care plan team members informed of resident assessment scheduling changes and status updates.
  • Review daily 24-hour reports and telephone orders for significant changes in resident conditions that might alter assessments and care plans.
  • Participate in facility surveys as necessary.
  • Complete resident assessments to capture the highest possible PDPM score.
  • Educate and train staff members about the assessment process.
  • Possess the ability to work as an RN, rounding with physicians as needed for optimum staffing.
  • Complete all MDS assessments and Care Area Assessment Summaries (CAAs).
  • Participate in the facility quality of care committees and meetings.
  • Ensure unit nurses complete quarterly special care assessments as required.
Required Skills
  • Relevant experience in clinical health care, including direct experience in MDS.
  • Ability to make independent decisions in a timely fashion when circumstances warrant.
  • Strong teamwork and collaboration skills; capable of contributing to a team environment.
  • Professional conduct with residents, families, staff, and external agencies; maintains ethical standards.
  • Leadership and supervisory ability; willingness to work harmoniously with other personnel.
  • Respects nursing profession boundaries and maintains accountability, integrity, and quality patient care.
  • Patience, tact, a cheerful disposition, and enthusiasm; able to handle difficult residents and staff.
  • Open to adopting new methods and principles and incorporating them into practice.
Required Experience
  • Education:

    Must have a minimum of a two-year degree.
  • Certification:
    Must be a Registered Nurse (RN).
Working Conditions /

Physical Requirements
  • Works in office areas as well as throughout the facility and premises.
  • Subject to frequent interruptions and interactions with residents, family members, personnel, and visitors.
  • May require working beyond normal hours, including weekends and holidays when necessary.
