Primecare Hospice seeks a registered nurse to plan, coordinate, and provide compassionate hospice care to patients in accordance with each patient’s plan of care.
Responsibilities include collaborating with the interdisciplinary group, evaluating care effectiveness, performing comprehensive assessments, and educating patients and families about end-of-life care and decision-making. On-call duties and a Monday–Friday schedule may be required.
