Overview
Social Services Director
Join our team at Ocean Grove Post Acute, a leading provider of short-term and long-term skilled rehabilitation services, as a Social Services Director
. Our facility in Millsboro, Delaware is seeking a motivated and compassionate professional to lead our social services team.
Salary Range: $65, per year based on experience
Responsibilities- Educating staff on the role of social services in patient care
- Developing and implementing individualized care plans for each patient s social service needs
- Conducting meetings with patients/residents, families, and significant others to provide support and information
- Discharge planning activities, including family education, nursing center documentation, and referral to community agencies and suppliers
- Establishing and maintaining relationships with community-based agencies and services
- A Bachelor s degree from an accredited school of Social Work or related field
- At least one year of supervised social work experience in a Skilled Nursing Facility, working directly with individuals (preferred)
- Licensed in Social Work for the state of PA
