As a Case Manager for the Community Support Services Program at Cumberland County Guidance Center, you will be at the forefront of advocacy and support for individuals with mental illness living in the community. You will help clients secure housing, maintain their integration into the community, foster their self‑sufficiency and independence, promote the use of vital community services, support clients in travel and fiscal management, and facilitate access to healthcare.

You will collaborate with treatment providers to develop tailored Individual Rehabilitation Plans, documenting progress to ensure effective service delivery. By assisting individuals in honing their social skills and daily living capabilities, you will empower them to thrive in their environment.

To excel as a Case Manager for the Community Support Services Program at Cumberland County Guidance Center, you will need the following qualifications:

• Bachelor's degree in a mental health‑related field or a Licensed Registered Nurse or Licensed Practical Nurse with relevant experience.
• Proficiency in community resources and agencies to connect clients with needed services.
• Strong communication and advocacy skills to build rapport with clients and collaborate with other treatment providers.
• Valid New Jersey driver's license with a clean five‑year driving record.
• Willingness to work a flexible 40‑hour schedule that may include evenings, weekends, holidays, and on‑call rotation.
