ANGELCARE HOME HEALTH SERVICES LLC is looking for a compassionate and skilled Physical Therapist to join our dedicated team in Vallejo, CA. This is a rewarding opportunity to make a meaningful difference in patients' lives by delivering high-quality, personalized care right in the comfort of their homes.
Responsibilities:- Evaluate patients' physical conditions, functional limitations, and rehabilitation needs through comprehensive assessments
- Develop and implement individualized physical therapy treatment plans to restore mobility and improve quality of life
- Provide therapeutic exercises, manual therapy, and other evidence-based PT interventions in a home health setting
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Educate patients and caregivers on home exercise programs, safety techniques, and injury prevention
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care
- Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards
- Valid California Physical Therapist (PT) license in good standing
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
- Home health or outpatient rehabilitation experience preferred
- Strong knowledge of Medicare home health guidelines and documentation requirements
- Excellent communication, interpersonal, and patient education skills
- Reliable transportation and valid driver's license for home visit travel
- CPR/BLS certification required or ability to obtain prior to start
