Schedule:
Monday - Friday
Pay Rate: $25.00 per hour
What You’ll Do- Conduct engaging patient assessments and develop personalized care plans.
- Collaborate with healthcare providers to deliver smooth and efficient care.
- Manage referrals and follow up with patients to ensure timely care.
- Coordinate communication between patients, providers, and family members—keeping everyone aligned.
- Provide support exclusively in the field!
- Demonstrate professionalism and excellent phone etiquette in every interaction.
- Partner with multiple providers across the Access Healthcare Associates network.
- Monitor patient progress and adjust care plans to fit individual needs.
- Educate and empower patients and families with resources and guidance.
- Apply resource management skills to optimize care delivery.
- Conduct diagnostic evaluations and recommend tailored treatment options.
- Support team goals with additional duties as needed
- Perform other duties as assigned to support patient care and team goals.
- You have a Medical Assistant Certificate (Required)
