Compensation: $135,000+ Annual Salary

Local Territory Only: Your patients are in Santa Clara County / South Bay
. You will never be asked to drive to San Francisco or the East Bay.

Financial PackageAnnual Salary: Starts at $135,000 per year (Increases based on experience & Case Management skills)
Performance Bonus: Earn an additional up to 25% of your annual wage based on quality-of-care metrics.
Overtime Eligible: Unlike many agencies, our clinicians are NOT Exempt. If you work late or take an extra shift, you get paid for it.

ResponsibilitiesProvide skilled nursing care in accordance with the patient's individual Plan of Care.
Collaborate with the Interdisciplinary Team (PT, OT, ST, MSW) to ensure holistic outcomes.
Supervise Licensed Vocational Nurses (LVNs) when assigned.
Evaluate patient progress and report changes to the Case Manager or Director of Nursing.
Complete documentation in a timely manner using our AI-powered charting software.
Requirements1+ year of nursing experience (Home Health preferred).
Valid Driver's License and reliable transportation.
Compassionate, self-driven, and ready to work independently.
