Community Liaison - Home Health

The Community Liaison is responsible for establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and negotiating service contracts with managed care organizations, government agencies, and other payers.

• Develop and maintain relationships with physicians, discharge planners, case managers, social workers, and other health care professionals by regularly scheduling and conducting visits to those referral sources.
• Assist in identifying patients appropriate for home health and hospice services, thereby increasing the number of clients served by the agency.
• Maintain current knowledge of agency and market trends, coverage criteria, and industry changes to ensure regulatory compliance.
• Represent the agency at health fairs, mall shows, community service functions, and civic committees as requested.
• Continuously conduct market assessments and develop comprehensive marketing plans that meet budgetary volume projections.
• Work with the Director of Business Development to establish marketing techniques and employ promotional initiatives to achieve volume targets.
• Build and monitor community, customer, payer, and patient perceptions of Sequoia Home Health as a high‑quality provider.
• Provide leadership in strategic planning to identify opportunities for additional or improved services that address customer needs.
• Assist customers in accessing community resources when services are not provided by Sequoia Home Health.
• Monitor and report the cost effectiveness of marketing efforts.

• Bachelor’s degree in Marketing, Business Administration, or a related field is preferred but not required.
• Minimum three years of experience in healthcare marketing management, ideally in home health operations.
• Ability to market aggressively while dealing tactfully with customers and the community.
• Knowledge of corporate business management.
• Strong communication, negotiation, and public relations skills.
• Demonstrated autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.

Pay Range: $80,000 - $150,000 per year.
