Each assessment typically lasts about 45 minutes and may include annual health checks or post-discharge visits. Schedule- Flexible shifts available Monday through Sunday
- Design your own schedule to balance personal and professional commitments
- Commit to your preferred minimum weekly or bi-weekly hours
- Complete annual home health risk assessments for Medicare Advantage members
- Review and document patients medical, surgical, and medication histories
- Deliver relevant health education regarding chronic and acute conditions
- Identify gaps in care and provide actionable recommendations
- No prescribing or treatment plan development required
- Active Nurse Practitioner license in Delaware
- Board certification via AANP or ANCC
- Valid BLS CPR certification
- MSN from an accredited nursing program
- At least 6 months of NP experience preferred
- $80 $ 90 per home health evaluation, reflective of your expertise
- Reimbursement for mileage and administrative time
