Emerald Coast Hospice is seeking an RN Case Manager to deliver compassionate, patient-centered hospice care in patients' homes and community settings across Florida. You will assess, plan, and coordinate care with an interdisciplinary team to ensure comfort, dignity, and support for patients and families.
Qualified candidates hold an active Florida RN license, have 1 year of RN experience (hospice/home health preferred), CPR certification, and a valid driver’s license.
