Center Well Home Health is seeking a dedicated Home Health Physical Therapist to support patients in their homes under supervision of the Clinical Manager. The role focuses on improving functional abilities, mobility and independence through individualized therapy plans.
You will deliver prescribed PT, assess patient function, guide goals with the physician, and supervise PTAs and Home Health Aides, documenting progress and coordinating with the care team to ensure optimal outcomes.
