Registered Nurse; RN - Home Health - per visit

Position Details

Full‑time Home Health Registered Nurse (RN) in Milton, Massachusetts. Territory includes Sharon, Canton, Norwood, Westwood, Dedham and Dover. Ongoing employment as a staff member.

• Use comprehensive head‑to‑toe assessment skills to identify subtle changes in patient condition.
• Provide acute and post‑acute care (CHF, COPD, wound care, IV therapy, post‑surgical care) in the home setting.
• Triage and manage urgent clinical situations independently.
• Perform medication reconciliation, teaching, and safety monitoring.
• Perform comprehensive wound assessment, care, and documentation.

• Ability to work independently while knowing when to escalates concerns.
• Comfortable making real‑time clinical decisions without immediate on‑site support.
• Strong problem‑solving skills in unpredictable home environments.
• Ability to manage a dynamic visit schedule and prioritize urgent visits.
• Efficient with point‑of‑care documentation and technology.
• Strong attention to detail while maintaining productivity expectations.
• Comfortable providing care in varied home environments.
• Flexible and adaptable to changing patient needs and schedules.
• Demonstrates cultural sensitivity and respect for patient autonomy.

Competitive pay ranges with a weekly pay schedule: $60.00 – $68.00 per point depending on qualifications and subject to negotiations. Flexible scheduling. Weekly base pay and a $10,000.00 sign‑on bonus.

• A current nursing license in good standing in the state of Massachusetts.
• Graduation from an accredited and approved nursing program (school transcript or diploma).
• At least one year of current or recent hands‑on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med‑surg, sub‑acute care or rehab units.
