Registered Nurse - Hospice

The Registered Nurse (Hospice) provides compassionate, patient-centered care to individuals and families facing end-of-life needs. This role ensures quality clinical care, emotional support, and coordination of services in alignment with hospice philosophy and standards.

• Deliver skilled nursing care to hospice patients in homes, facilities, or inpatient settings.
• Assess patient conditions and develop individualized care plans in collaboration with the interdisciplinary team.
• Administer medications and treatments as prescribed.
• Provide education and emotional support to patients and families regarding disease process, symptom management, and end-of-life care.
• Coordinate care with physicians, social workers, chaplains, and other team members.
• Document patient assessments, interventions, and outcomes in compliance with regulatory and organizational standards.
• Participate in on-call rotation to ensure continuous patient support.

• Current Registered Nurse (RN) license in the state of practice.
• Must have at least 2 years of recent experience in the specialty within the past 2 years.
• Strong communication and interpersonal skills.
• Ability to work independently and manage multiple patients.
• Commitment to providing compassionate, dignified care.

• Prior experience in hospice or palliative care.
• Familiarity with electronic health record (EHR) systems.
• Training in pain management and symptom control.

• Basic Life Support (BLS) certification required.
• Hospice and Palliative Care Nursing Certification (CHPN) preferred.
