Champion Care is growing and looking for an experienced Admissions Director to join our centralized skilled nursing admissions team located at our corporate regional office in Milwaukee, Wisconsin.

This is a fully offsite admissions role working from our central admissions office. This position is ideal for someone with skilled nursing facility (SNF) admissions experience who thrives in a fast-paced, process-driven environment and enjoys coordinating seamless patient transitions behind the scenes.

Qualified candidates must have prior SNF admissions experience.

What You’ll Do

Skilled Nursing Admissions & Referral Management

• Manage incoming SNF referrals

• Review and process referral documentation, insurance information, and supporting clinical records

• Coordinate financial and clinical admissions reviews with internal team members

• Communicate with hospitals, case managers, discharge planners, and referral partners

• Maintain accurate admission tracking and documentation throughout the process

Insurance Authorizations & Coordination

• Manage payer authorizations, including Medicaid Managed Care plans

• Obtain required clinical and case management documentation

• Upload and maintain documentation within PCC and related systems

• Coordinate admission approvals and communicate updates to admitting facilities

• Ensure equipment, transportation, and admission logistics are arranged and ready

Qualifications

• High school diploma or equivalent required

• Previous skilled nursing facility (SNF) admissions experience required

• Experience with healthcare admissions, intake coordination, case management, or insurance authorization preferred

• Experience with Census Pro, PCC, and insurance authorization portals strongly preferred
