Job Summary

This position provides case management to families involved in the child welfare system due to identified child safety concerns. Drawing upon strength‑based, trauma‑informed practice and utilizing a family teaming model, works in partnership with parents and their family team to manage child safety while enhancing parents' ability to keep their children safe, with a primary goal of keeping families intact and achieving safe reunification.

Responsible for safety assessment, safety planning in in‑home and out‑of‑home care placement settings, and case planning utilizing both formal service providers and informal supports. Utilizing critical thinking skills, case managers facilitate and monitor case progress towards permanency and safe case closure.

Essential Functions
  • Engages clients and focuses on establishing trusting relationships with parent clients and their children with the purpose of engaging and supporting parents through the case planning and behavioral change process.
  • Assesses safety and determines plans in conjunction with case/permanency planning and service coordination to ensure child safety and child and family well‑being.
  • Coordinates a multi‑disciplinary family team comprised of formal and informal supports, maintains ongoing communication with team members, and facilitates family team meetings to monitor case progress.
  • Conducts in‑home and community‑based visits with children, families, and caregivers.
  • Facilitates placement into out‑of‑home care settings, establishes family interaction plans, and supports caregivers in meeting the needs of children and youth in partnership with their families.
  • Documents case progress in the case record and completes court documentation.
  • Understands the role of Federal, State and tribal legislation as related to the Division of Milwaukee Child Protective Services in day‑to‑day planning, protection and intervention for children and families.
  • Collaborates with legal parties and represents the child welfare perspective in court proceedings, providing testimony as needed.
Education
  • Bachelor’s Degree in Social Work, Psychology, Sociology, Criminal Justice or a related degree that meets child‑placing license requirements.
Experience
  • 1+ years of experience in the delivery of educational training, employment services, vocational counseling, social services or a related field.
  • Case management experience.
  • Experience working with families of diverse ethnic, cultural and socioeconomic backgrounds strongly preferred.
Knowledge,

Skills and Abilities
  • Acceptable driving record, personal auto liability insurance coverage and the ability to be insured by the CHHS auto insurance carrier.
  • Ability to engage non‑voluntary clients.
  • Ability to work with diverse groups in a culturally competent manner.
  • Organization and time management skills.
  • Effective verbal, written and interpersonal communication skills.
