Physical Therapist

Position Overview

Inabia is seeking a licensed and motivated Physical Therapist to join its Rehab Therapy team. This role focuses on delivering high-quality outpatient physical therapy services to help patients restore function, improve mobility, and achieve optimal outcomes.

The ideal candidate is patient-focused, clinically strong, and committed to providing individualized care in a collaborative clinical environment.

• Evaluate patients’ physical conditions, mobility, strength, and functional limitations

• Develop and implement individualized treatment plans based on patient needs and goals

• Provide hands-on therapy, therapeutic exercises, and patient education

• Monitor patient progress and adjust treatment plans as needed

• Document evaluations, progress notes, and discharge summaries accurately and timely

• Collaborate with physicians and interdisciplinary team members

• Maintain compliance with clinic policies, state regulations, and professional standards

• Schedule: 5 × 8-hour day shifts

• Consistent weekday schedule

• Supportive clinical environment with opportunities for professional growth

• Active Physical Therapist license in the state of Wisconsin (or ability to obtain)

• Degree in Physical Therapy from an accredited program

• Outpatient or rehab experience preferred but not required

• Strong communication, documentation, and patient-care skills
