This ONSITE Clinical Coordinator coordinates the daily Clinical Hospice Team functions from the office to assure effective coverage of all Team clients. Acts as a resource for field staff, including Therapists, Case Managers, Patient Care Coordinators, Nurses, Social Workers, Chaplains, and Home Health Aides, while functioning as a liaison that represents the team with all internal and external customers. Provides services at quality levels consistent with professional standards of practice.
Responsibilities- Ensure appropriate staffing and coordination for all team patients, and supports the timely processing of all related paperwork.

- Participates in holiday office coverage rotation.

- One Year college/vocational/technical School One.
- (1) year experience as appropriate for team clerical functions.
- Working knowledge of Home Care computer system with demonstrated keyboarding proficiency preferred.
- Current/Valid WI Drivers License; plus reliable/insured vehicle.
Full-time, Monday-Friday 8am-4:30pm
