: $95,000/year based on experience | Sign-On Bonus included

Schedule:Monday – Friday, 8:00 AM – 5:00 PM (flexible start and end times)

The MDS Coordinator plays a vital leadership role in our skilled nursing community. This is more than a compliance position—it is a strategic clinical role that directly impacts resident outcomes, interdisciplinary care planning, reimbursement accuracy, and survey readiness. You will not be working in isolation—you will partner closely with nursing leadership, therapy, social services, and administration in a culture that values accountability, teamwork, and respect.

What You Will Do:
  • Oversee and coordinate the MDS process for resident assessments and care plans.
  • Coordinate and complete accurate, timely MDS assessments
  • Ensure compliance with all federal and state regulations.
  • Collaborate with the interdisciplinary team to develop and update comprehensive, resident-centered care plans.
  • Monitor clinical documentation to ensure accuracy and optimal reimbursement.
  • Manage assessment schedules and submission deadlines with organizational precision.
  • Support survey readiness and participate in quality improvement initiatives.
  • Provide education and guidance to nursing staff regarding documentation standards.
  • Serve as a clinical resource for reimbursement processes and regulatory compliance.
  • Collaborate with interdisciplinary team members to implement comprehensive care plans.
What You Will Bring:
  • Active RN license (required).
  • Proven experience as an MDS Coordinator in skilled nursing or long-term care. (minimum of 2 year MDS experience required)
  • Current RN license in the state of Wisconsin.
  • Experience preparing for and navigating state surveys.
  • Exceptional attention to detail and strong organizational skills.
  • Ability to work independently while collaborating effectively with interdisciplinary teams.
  • Strong communication skills and confidence in clinical decision‑making.
