Position Summary

The Case Manager provides comprehensive, person-centered case management and care coordination services to individuals served by the Wisconsin Mobile Mental Health Unit. The position conducts psychosocial and needs assessments, assists with crisis stabilization, develops individualized service plans, coordinates referrals, provides brief supportive interventions, facilitates psycho educational activities, and connects individuals with behavioral health, medical, housing, financial, and social supports. The Case Manager works in community-based settings and is responsible for safely operating the Mobile Mental Health Unit, transporting clients as appropriate, conducting outreach, and supporting engagement in ongoing treatment and long-term stability.

ESSENTIAL JOB FUNCTIONS
  • Engage individuals experiencing behavioral health concerns or crises using a respectful, trauma-informed, person-centered approach.
  • Conduct psychosocial, functional, and service-needs assessments and identify immediate barriers to safety, stabilization, treatment, and recovery.
  • Assist the clinical team with crisis stabilization activities and implementation of individualized safety, service, and follow-up plans.
  • Develop, implement, monitor, and update individualized service plans in collaboration with the individual, treatment team, family or natural supports when appropriate, and community providers.
  • Provide case management, care coordination, brief supportive interventions, education, advocacy, and problem-solving assistance within the scope of the position.
  • Coordinate referrals and warm handoffs to behavioral health, substance use, medical, housing, benefits, employment, transportation, financial, and other community-based services.
  • Follow up on referrals and services to support continuity of care, reduce barriers to treatment, and promote successful engagement after a crisis encounter.
  • Conduct community outreach and engagement activities with individuals who may have difficulty accessing or remaining connected to services.
  • Facilitate or assist with psycho educational, wellness, recovery, and skill-building groups as assigned.
  • Maintain timely, accurate, objective, and complete documentation of assessments, contacts, referrals, services, follow-up activities, and outcomes in accordance with program and organizational requirements.
  • Communicate significant changes in an individual's condition, safety, functioning, or service needs to the Clinical Director and appropriate members of the treatment team.
  • Participate in interdisciplinary case consultation, care coordination meetings, staff meetings, supervision, training, and quality-improvement activities.
  • Maintain current knowledge of community resources and develop collaborative working relationships with hospitals, behavioral health providers, social service agencies, shelters, law enforcement or first responders when applicable, and other referral partners.
  • Safely operate the Mobile Mental Health Unit and other authorized program vehicles; complete required vehicle checks and follow organizational transportation and safety procedures.
  • Transport clients to services, appointments, other approved destinations when appropriate and authorized by program procedure.
  • Protect client confidentiality and handle protected information in accordance with organizational policy and applicable privacy requirements.
