Overview

The Medical College of Wisconsin’s Department of Medicine, Division of General Internal Medicine, is seeking a Nurse Practitioner or Physician Assistant to join the Primary Care Outpatient Services at Froedtert Hospital Specialty Clinics in Milwaukee, Wisconsin. The APP will function as a member of a multidisciplinary team and contribute to excellence in patient care, safety, research, quality improvement, teaching, and program administration.

Our Advanced Practice Providers (APPs) work closely with physicians to deliver coordinated, team-based care. Join a group that provides comprehensive outpatient services grounded in scientific knowledge, clinical expertise, and compassionate care for adults—including those with multiple chronic medical conditions. Our providers help patients understand disease prevention and manage conditions such as cardiovascular disease, endocrine disorders, cancer, and musculoskeletal disease. We also support patients with chronic illnesses by coordinating care with specialty providers.

Through collaboration with nursing staff, social workers, and community health workers, we deliver comprehensive and coordinated care. The ideal candidate is an experienced Primary Care APP who seeks to work in a collaborative academic environment.

In This Role, You Will
  • Collaborate with an interdisciplinary team in diagnosing and treating conditions requiring general internal medicine intervention in an outpatient setting.

  • Write accurate and timely progress notes, procedure notes, and clinical documentation.
  • Facilitate communication with patients, families, and staff to promote continuity of care.
  • Use evidence-based knowledge to guide clinical decision-making for wellness, women’s health, and acute and chronic conditions.
  • Conduct post-hospital and emergency department follow-up visits.
Core Advanced Practice Responsibilities
  • Perform comprehensive histories and physical exams, identifying normal and abnormal findings across physical, cognitive, and psychosocial domains.
  • Order, perform, and interpret diagnostic tests, and communicate results appropriately.
  • Develop and monitor pharmaceutical and non-pharmaceutical treatment plans.
  • Educate patients on disease processes and treatment options.
  • Perform procedures as trained and credentialed.
  • Document care in the Electronic Medical Record (EMR) and complete required records.
  • Collaborate and consult with physicians and other healthcare providers as needed.
  • Counsel patients and families on health maintenance and preventive care.
  • Attend required training and meet institutional and regulatory requirements, including CMS and Joint Commission standards.
