Position DetailsFull Time, 1.0 FTE
Standard
Hours:
40 hours per week
Shift 1:
Monday‑Friday, 8:00 am‑4:30 pm (no holidays or weekends)
Provides primary clinical management and coordination of care for cystic fibrosis patients. Coordinates care across the continuum for this pulmonary population, working with patients, families, providers, social workers, support staff, inpatient colleagues, third‑party payors, and community resources. May also provide services within Nursing Scope of Practice and coordinate quality improvement initiatives for this patient population.
ExperienceThree to five years of nursing experience required. Experience in an acute care, intensive care, or ambulatory clinic setting preferred.
EducationBachelor's degree in Nursing required.
Special SkillsHigh degree of organizational skills, excellent communication, independent work, programmatic initiative, problem solving, and proactivity in assisting the team to manage the population.
LicensureCurrent Wisconsin Registered Nurse License or a Multi‑state Nursing License from a participating state in the NLC. AHA Basic Life Support for Healthcare Providers (BLS) or an AHA‑approved equivalent required within 90 days.
