Overview
Heartland Home Health, a Gentiva company, is seeking a Full‑Time Registered Nurse to provide skilled home‑health care to patients throughout Milwaukee, WI. This role offers an opportunity to build meaningful connections and help patients maintain safety and independence at home.

Monday – Friday - RN Licensure:
Wisconsin - Sign‑On Bonus: $7,500 (available)

- Serve as the primary nurse for assigned patients, managing individualized Plans of Care and supporting patient safety and independence in the home.
- Provide and document skilled nursing care, including medication administration, wound care, and patient education.
- Conduct comprehensive assessments and ongoing reassessments to monitor progress and adjust care plans as needed.
- Act as the main liaison among the physician, patient/family, and the home‑health team.
- Coordinate care with physical therapists, occupational therapists, social workers, home health aides, and other interdisciplinary team members.
- Educate patients and families on health management, safety precautions, and self‑care techniques.
- Communicate patient status updates and care needs to the team and physicians to ensure timely interventions.
- Support the organization’s quality initiatives and maintain accurate, compliant clinical documentation.
- Active RN license in the state of employment (or eligibility to obtain).
- Minimum of 1+ year of high‑acuity nursing experience.
- Compassionate, flexible, and patient‑centered approach to care.
- Comfort providing care in diverse home and community settings.
- Strong organizational, communication, and clinical decision‑making skills.
- Valid driver’s license, automobile insurance, and reliable transportation.
- Current CPR certification.
- 1+ year experience in home health, hospice, ICU, geriatrics, Med‑Surg, ER, or related clinical settings.
- Admissions RN, wound care, float pool, or travel nursing experience.
