Job OverviewThe APP will function as a member of a multidisciplinary team within the Primary Care outpatient clinic. The role supports comprehensive outpatient services for adult patients with chronic medical conditions, provides education, coordination of care, and participation in new team‑based programs in Women’s Health, Obesity Management, and Cancer Survivorship.
Responsibilities- Collaborate with an interdisciplinary team in diagnosing and treating conditions requiring general internal medicine intervention in an outpatient setting.
- Manage primary care needs, acute concerns, and co‑morbid conditions such as diabetes, hypertension, heart failure, asthma, and anemia.
- Write accurate and timely progress notes, procedure notes, and clinical documentation.
- Facilitate communication with patients, families, and staff to promote continuity of care.
- Use evidence‑based knowledge to guide clinical decision‑making for wellness, women’s health, and acute and chronic conditions.
- Conduct post‑hospital and emergency department follow‑up visits.
- Perform comprehensive histories and physical exams, identifying normal and abnormal findings across physical, cognitive, and psychosocial domains.
- Order, perform, and interpret diagnostic tests, and communicate results appropriately.
- Develop and monitor pharmaceutical and non‑pharmaceutical treatment plans.
- Educate patients on disease processes and treatment options.
- Perform procedures as trained and credentialed.
- Document care in the Electronic Medical Record (EMR) and complete required records.
- Collaborate and consult with physicians and other healthcare providers as needed.
- Counsel patients and families on health maintenance and preventive care.
- Attend required training and meet institutional and regulatory requirements, including CMS and Joint Commission standards.
- Stay current with developments and trends in the Nurse Practitioner or Physician Assistant profession through education and professional engagement.
- Preferred 2+ years of APP Primary Care experience; 5+ years of APP experience is a plus.
- Prior experience in cancer care, survivorship, or oncology primary care (as APP or RN).
- Familiarity with hereditary cancer syndromes and cancer after‑care programs.
- Ability to collaborate with oncology teams and manage complex cancer survivor populations.
- Minimum Education:
- Advanced Practice Nurse – Master’s Degree or higher in Nursing from a nationally accredited organization.
- Physician Assistant – Bachelor’s Degree or higher in Physician Assistant Studies from an ARC‑PA‑accredited organization.
- Licensure and Certification Requirements:
- Wisconsin RN License (for NP).
- Wisconsin Advanced Practice Nurse Prescriber License (for NP).
- Wisconsin Physician Assistant License.
- American Heart Association Advanced Cardiac Life Support (ACLS).
- American Heart Association Basic Life Support (BLS).
- DEA Registration.
- NP certifications:
Adult‑Gerontology Acute Care NP (AGACNP), Adult‑Gerontology Primary Care NP (AGPCNP), Family NP (FNP). - Physician Assistant Certification by the National Commission on Certification of Physician Assistants (NCCPA).
Full‑time (1.0
FTE), Monday–Friday with coverage during business hours. Salaried (Exempt).
