The RN Case Manager coordinates care and services of selected patient populations through the acute care episode and across the continuum to support an effective discharge plan. The RN Case Manager works collaboratively with interdisciplinary teams both internal and external to the organization to improve patient care by assuming a leadership role in driving desired clinical and financial outcomes through effective utilization and monitoring of healthcare resources.
The RN Case Manager utilizes standards, guidelines, and protocols for care delivery and incorporates data to continuously improve care and outcomes.
- Coordinate care and services for selected patient populations across the continuum.

Minimum 3 years of clinical nursing required; 5 years of clinical nursing and case management experience preferred. Experience in case management including transitional planning, outcomes management, and assessment preferred.
EducationProfessional knowledge of nursing theory and practice normally acquired through a minimum of two years of education at an accredited School of Nursing. Bachelor's degree preferred.
TrainingAccredited case manager certification (ACM-RN) through the American Case Management Association preferred.
Special Skills- General knowledge of payor industry, resource management, and evidence‑based clinical practice.
- Theoretical knowledge of nursing and case management processes.
- Analytical skills to assess patients' discharge needs and develop solutions.
- Advanced communication and interpersonal skills with all levels of internal and external customers.
- Excellent written and verbal communication, critical thinking, creative problem solving, organization, and planning.
- Basic computer skills.
- Self‑directed, ability to tolerate frequent interruptions and demanding workload.
Requires current Wisconsin Registered Nurse License or Multi‑state Nursing License from a participating state in the NLC.
Shift DetailsShift 1:
Monday‑Friday.
US: WI: MILWAUKEE at Froedtert Hospital facility.
