Registered Nurse Care Coordinator; Home Health Care RN

Position Summary

Registered Nurse is responsible for the supervision of personal care workers and the delivery of personal care services to consumers. Work duties include home visits, consumer assessments, development of appropriate plans of care, and supervision of and performance evaluation of personal care workers. Reports to the Manager of Personal Assistance Services.

• Supervise personal care workers.
• Conduct home visits and consumer assessments.
• Develop and implement plans of care.
• Evaluate performance of personal care workers.

• Graduate of an accredited school of nursing.
• Valid license as Registered Nurse where the assigned facility is located.
• Valid driver's license with a good driving record.
• One year supervisory experience.

• Full‑time, Monday‑Friday 8:00 am – 4:30 pm, hybrid schedule (no weekends or holidays).
