Position: Full-time contract for primary care/acute care APP (Evening Hours)

Job Overview

Join our team committed to delivering the highest quality healthcare service. We are seeking a Full‑time Contract APP for primary care/acute care (Evening Hours) who is self‑motivated, energetic, and a take‑charge individual. Spanish speaking preferred.

Job Responsibilities
  • Provide limited care coordination and acute care (e.g., laceration repair, point‑of‑care testing, asthma management, immunization updates, simple splinting) to clinic patients ranging from newborns to adults in collaboration with other medical providers.
  • Evaluate, interpret, diagnose, and prescribe appropriate medical and therapeutic measures for both acute and chronic conditions.
  • Act as the primary patient contact, ensuring continuity of care and smooth transitions between care settings, including post‑discharge plans.
  • Offer clinical direction to nursing staff and follow up to ensure recommended care is delivered.
  • Prescribe, recommend, and regulate pharmacologic and non‑pharmacologic therapies, monitoring controlled medication use and pain contracts; limit narcotic prescribing.
  • Coordinate ancillary services, internal or external referrals, and school‑based clinic communication for school‑patient referrals.
  • Order, perform, and interpret procedures and diagnostic tests within scope (e.g., injections, immunizations, suturing, wound care, biopsies).
  • Counsel, instruct, and educate patients on health behaviors, disease management, treatment options, and developmental milestones.
  • Interpret and evaluate patient data, consult with physicians as appropriate, and adjust care plans accordingly.
  • Prepare and sign patient care plans, encounter notes, and summaries; maintain records per established standards and guidelines.
Scheduled Hours

FTE .7 – 1.0 (negotiable). Set number of 4‑hour shifts per week scheduled based on department needs, with at least three evening shifts (4–8 pm). Shifts are scheduled Mon‑Fri 8 am–8 pm in 4‑hour blocks. Clinic hours are Mon‑Fri, 8 am–8 pm with shifts 8–12, 12–4, and/or 4–8.

Licenses & Certifications
  • Physician Assistant or Nurse Practitioner license issued by the state in which the team member practices.
  • Family Nurse Practitioner certification (AANPCB) or Physician Assistant (PA‑C) certification (NCCPA).
  • DEA registration.
  • Basic Life Support (BLS) certification issued by the American Heart Association (AHA) within 6 months.
Degrees
  • Master’s Degree in Physician Assistant Studies (may consider Bachelor’s with 7+ years of experience).
  • Master’s/Doctorate Nursing Degree in Family Practice/Primary Care.
Required Functional Experience

Typically requires 2+ years of experience providing and coordinating care for patients with acute and chronic health conditions as a physician assistant or nurse practitioner.
