Schedule: pick up shifts as needed, must pick up a minimum of two shifts per month

We are searching for a dedicated and caring Licensed Practical Nurse to join our exceptional team. In this role, you’ll be at the heart of providing top-notch care to our residents, ensuring their comfort and well-being with your expertise and compassion. You'll work alongside a dynamic and supportive team of LPNs, RNs, CNAs, Dietary Aides, Physical Therapists, and Activities Staff, reporting directly to our Nurse Managers and Director of Nursing.

• Administer Treatments & Care
• Monitor Vital Signs
• Maintain Documentation
• Supervise & Support Staff
• Serve as a Resource
• Report Resident Care
• Medication Management
• Address Medication Concerns
• Respond to Call Lights
• Admit and Discharge Residents

• Active LPN license registered through the State of Wisconsin.
• Associate's or Bachelor’s degree in a related field
• CPR certification required
• Previous experience in Long Term Care and Skilled Rehabilitation is preferred
• Strong verbal and written communication skills
• Ability to work collaboratively in a team environment
• Demonstrate reliability and accountability
• A strong commitment to providing high-quality care
• Excellent interpersonal skills
• Ability to give and take instructions in a professional manner
• Ability to provide guidance and support to others
• A passion for resident care, empathy, and leadership skills
