Job SummaryDialysis Chronic Care Registered Nurse (RN) with 12 months or more of recent dialysis experience required. The nurse will be responsible for providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients, and will collaborate with the End Stage Kidney Disease (ESKD) healthcare team on decision-making, teaching, leadership, and quality improvement activities.
Purpose and ScopeAccountable for the provision and coordination of clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. Responsibilities include delegating appropriate tasks to direct patient care staff, participating in team meetings, and contributing to quality improvement activities that enhance patient care outcomes and facility operations.
Principal Duties and Responsibilities- Perform all essential nursing functions under the direction of the supervisor and guidance of the clinical educator, preceptor, or other registered nurses.
- Collect and analyze patient data before, during, and after hemodialysis treatment, document findings, adjust treatment plans as needed, and notify appropriate team members.
- Assess, collaborate, and document patient/family educational needs to provide initial and ongoing education.
- Direct and collaborate with direct and ancillary patient care staff to safely deliver hemodialysis therapy to assigned patients.
- Administer medications as prescribed or per approved algorithms, documenting justification and effectiveness.
- Initiate or assist with emergency response measures.
- Serve as a resource for the healthcare team, participate in staff training, and orient new staff as assigned.
- Ensure correct laboratory collection, processing, and shipping procedures are performed and reschedule missed or insufficient collections.
- Identify expected outcomes, update nursing assessment and plan of care for assigned patients through interdisciplinary collaboration.
- Ensure patient awareness of transplant and treatment modality options.
- Perform any other duties assigned by the supervisor.
The role involves heavy lifting, moving patients, aiding ambulation, and frequent prolonged standing. The nurse must be able to bend over, lift up to 200 lbs. with assistance, and lift chemical and water solutions of up to 30 lbs. up to 5 feet. Working environment is a healthcare facility with controlled temperature and moderate noise; exposure to infectious materials may occur.
The position may also require desk work and occasional travel to training sites or other facilities.
Candidates are required to take the Ishihara Color Blindness test as a condition of employment. Failure does not preclude employment; reasonable accommodation may be considered.
SupervisionTeam Leader or designated nurse in charge oversees Patient Care Technicians, LPNs, LVNs, and RNs after meeting the following: successful completion of all client education and training requirements for new employees, minimum 9 months of RN experience, and minimum 3 months of chronic/acute hemodialysis experience.
Education and Licensure- Graduate of an accredited School of Nursing.
- Current appropriate state licensure.
- Current or successful completion of CPR BLS Certification.
- Meet state practice requirements.
- 1 year of nephrology nursing experience within the last two years as an RN.
- Chronic hemodialysis experience required.
- Must float to area sites within 50 miles of the main site.
- Commitment of 40 hours per week plus overtime and floating; minimum guarantee of 36 hours per week. Guaranteed hours are not provided if schedule restrictions, refusal to float, tardiness, early exit, call-outs, or planned time off occur.
Commencing 5/25/2026, 14-week onsite contract. Works 8 hours/day, 5 days/week. Shift details will be discussed during interview.
