The Certified Nursing Assistant provides direct patient care under the supervision of licensed nursing staff, assisting patients with activities of daily living, monitoring health status, and maintaining a safe and comfortable environment for patients in their homes.
Qualifications
- Current certification as a Nursing Assistant in Wisconsin
- High school diploma or equivalent
- Valid driver's license and reliable transportation
- Current CPR and First Aid certification
- Successful completion of Wis Caregiver Careers program
- Physical ability to perform essential job functions
- Previous experience in home health care
- Experience with electronic health records
- Additional healthcare certifications
- Bilingual skills
Essential Duties and Responsibilities
- Assist patients with activities of daily living (ADLs)
- Perform personal care tasks according to care plan
- Monitor and record vital signs
- Observe and report changes in patient condition
- Assist with ambulation and transfers
- Follow infection control protocols
- Document care provided accurately and timely
- Report changes in patient condition to supervisor
- Complete required forms and reports
- Maintain patient confidentiality
- Maintain clean and safe environment
- Follow proper body mechanics
- Use appropriate personal protective equipment
- Report safety concerns promptly
- Interact professionally with patients and families
- Report to supervising nurse as required
- Participate in care plan meetings as requested
- Communicate effectively with team members
