Coverage Area: Travel throughout the territory is required – primarily Washington, Ozaukee, northeast Milwaukee Counties
Schedule: Full Time Monday–Friday 8AM‑5PM

- Assess and monitor physical, emotional, and psychological needs of patients
- Create hospice care plans that align with the patient's wishes and goals
- Direct nursing care: administer medications, treatments, and interventions
- Provide pain and symptom management
- Educate and support the patient's family and caregivers
- Collaborate with interdisciplinary team
- Maintain accurate and timely documentation
- Participate in on‑call rotation as required by local branch
- Graduate of an accredited nursing school with current licensure to practice in the state of operation
- One year nursing experience required, 3+ years preferred
- Experience in hospice or a similar setting preferred
- Knowledge of the hospice philosophy of care
- Commitment to clinical and documentation excellence
- Must be a licensed driver with an insured automobile that is in good working order as required by state or organization
