Location:

168 W Est 22730 Prairie View Ln, Jackson, WI 53037

Coverage Area: Travel throughout the territory is required – primarily Washington, Ozaukee, northeast Milwaukee Counties

Schedule: Full Time Monday–Friday 8AM‑5PM

Responsibilities
  • Assess and monitor physical, emotional, and psychological needs of patients
  • Create hospice care plans that align with the patient's wishes and goals
  • Direct nursing care: administer medications, treatments, and interventions
  • Provide pain and symptom management
  • Educate and support the patient's family and caregivers
  • Collaborate with interdisciplinary team
  • Maintain accurate and timely documentation
  • Participate in on‑call rotation as required by local branch
Qualifications
  • Graduate of an accredited nursing school with current licensure to practice in the state of operation
  • One year nursing experience required, 3+ years preferred
  • Experience in hospice or a similar setting preferred
  • Knowledge of the hospice philosophy of care
  • Commitment to clinical and documentation excellence
  • Must be a licensed driver with an insured automobile that is in good working order as required by state or organization
