Community Care, Inc. is seeking a Nurse Practitioner for our Pace Program in Milwaukee and Racine/Kenosha. This full-time role includes a $10,000 sign-on bonus and requires Wisconsin RN license, NP certification, and DEA/NPI credentials. Expect 40 hours weekly, M-F, with on-call weeks.
Ideal candidates have at least 1 year of clinical nursing experience with at-risk or elderly populations and are committed to helping adults live independently in the community.
