RN – Hospice Weekend Baylor job at Horizon Home Care & Hospice – Milwaukee, WI.

Weekends Only – Every Saturday and Sunday, 8:00 AM – 8:00 PM.

Salary:
Exempt – Full Time.

• Assess patient symptoms and family/caregiver situation and provide appropriate clinical intervention with the supportive hospice team for patients needing a weekend visit.
• Identify and provide for patient and family's educational needs.
• Use agency-provided laptops, cell phones, portable printers, bag, and fleet car (PRN).
• Communicate patient issues and concerns to the Care Manager / Patient Care Coordinator, appropriate health care professional, IDG (Hospice only), and/or Clinical Manager.

• Degree from an accredited school of nursing (BSN preferred).
• Current WI RN license.
• 1-2 years of hospice, oncology, or related experience.
• Experience in the care of the terminally ill; knowledge of hospice care.
• Hospice certification is preferred.
• Current/valid WI driver's license and reliable/insured vehicle.
