Job Summary:

Are you an adventurous and compassionate Registered Nurse (RN) looking for a new challenge? We have an exciting opportunity for a Travel RN MDS to join our team at Champion Care in Wisconsin. As a valued member of our team, you will provide exceptional care to our residents while traveling to various skilled nursing and rehabilitation communities within Wisconsin.

About the Role:

As a Travel RN MDS, you will be responsible for coordinating and conducting thorough assessments of residents' medical needs, developing and implementing individualized care plans, and ensuring that residents receive the highest level of care. You will work closely with our interdisciplinary team to develop and implement effective care plans, as well as provide education and support to residents, families, and healthcare professionals.

Responsibilities:

  • Conduct thorough assessments of residents' medical needs, including physical, emotional, and social factors

  • Develop and implement individualized care plans that address residents' unique needs and goals

  • Provide direct patient care, including administering medications, wound care, and other treatments

  • Collaborate with the interdisciplinary team to develop and implement effective care plans

  • Educate residents, families, and healthcare professionals on care plans and treatment options

  • Maintain accurate and detailed records of resident assessments, care plans, and treatment outcomes

  • Ensure compliance with regulatory requirements and company policies

  • Participate in quality improvement initiatives to improve resident outcomes and care processes

Ideal Candidate:

  • Travel RN MDS with a current RN license and experience in skilled nursing and rehabilitation communities

  • Strong assessment and evaluation skills, with the ability to develop and implement effective care plans

  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals

  • Ability to work independently and as part of a team to achieve common goals

  • Strong attention to detail and ability to maintain accurate records
