  • Ensure smooth transitions between care settings according to critical pathways
  • Develop relationships with patients and primary care physicians to facilitate cancer diagnosis, treatment, and follow-up
  • Personalize service management for patients with cancer
  • Complete nursing assessments focused on accountable disease sites and identify individualized patient needs
  • Integrate with departments and disciplines to develop plans of care
  • Guide patients with suspected cancer through the diagnostic phase
  • Track testing and diagnostic reports to ensure timeliness
  • Provide biopsy procedure follow-up phone calls as needed
  • Communicate and collaborate with physicians, office staff, hospital departments, healthcare providers, disciplines, and community agencies
  • Refer patients to financial counseling, cancer counseling services, and clinical trial research programs
  • Advocate for patients with cancer and maintain contact with those requiring ongoing support
  • Identify patients with compromised coping skills or lack of social support
  • Facilitate support groups for patients with cancer and their families/significant others
  • Develop optimal caregiver systems to achieve effective patient/family outcomes
  • Provide staff in-services and education offerings
  • Assist leadership with interviews, orientation, and staff/student nurse training
  • Counsel and educate patients and families about cancer prevention and early detection
  • Evaluate or design patient education materials for specific disease-site programs
  • Promote awareness of cancer disease-site programs through community education and outreach
  • Provide workshops, seminars, and presentations to professional and lay audiences
  • Partner with cancer-services leadership to develop program vision, goals, and objectives
  • Develop, implement, and maintain quality programs evaluating costs, patient care, and customer satisfaction
  • Utilize data to implement best practices
  • Analyze variances in NCCN treatment guidelines and algorithms to improve quality and efficiency, control costs, and systematize total patient care
Requirements
  • Registered Nurse license issued by the state in which the team member practices
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required
  • Associate's Degree in Nursing
  • Typically requires 5 years of experience in clinical nursing
  • Working knowledge of the components of quality and acute patient care needs specifically related to the area/function in which care management will be performed
  • Excellent analytical and interpersonal communication skills necessary to negotiate with families, patient, physicians and third party payers for optimum plan
  • Demonstrated ability to work well with physicians and other professionals in a direct and positive manner
  • Demonstrated ability to operate all equipment necessary to perform the job, i.e., FAX, computer, copier, telephone system
  • Knowledge and skills necessary to provide care appropriate to the age of the patients served
  • Ability to assess data reflective of the patient's status and interpret appropriate information to identify age-specific needs
  • Manual dexterity required for operation computer and calculator
  • Visual acuity required to facilitate review of written documents/computer screens, medical records, and to record information accurately
  • Clear oral communications and hearing acuity required for receiving instructions and converse on standard telephone
  • Functional speech and hearing to allow for effective communication of instructions and conversation over the telephone
