Care Coordinator (Wraparound)

About this positionWorking under the Wraparound Practice Model of care, which emphasizes individualized, strength‑based, trauma‑informed, community‑based, and culturally intelligent care, the Care Coordinator provides intensive and individualized care coordination for children and adolescents.
Responsibilities- Maintain a caseload of approximately 8 children and families.
- Contact enrolled children and families within 48 hours of enrollment and schedule an initial visit within 5 business days.
- Schedule and attend weekly visits, facilitate monthly Child and Family Team meetings, and maintain regular phone contact with families and service providers.
- Assemble a Child and Family Team within 2 weeks of enrollment.
- Within the first 30 days, coordinate the meeting of the child and family team, develop an individualized Plan of Care and a comprehensive 24‑hour Crisis Plan.
- Monitor the provision and quality of services, acting as liaison when new services or resources need to be sought or developed.
- Advocate for children and families across home, educational, court, and community settings as needed and attend all scheduled court hearings.
- Seek community resources with the team and modify the SAR when services or resources change.
- Provide or arrange transportation for youth/young adults to appointments or crisis/respite services.
- Provide or secure support and crisis/emergency services for enrolled youths and/or their families.
- Complete required paperwork accurately (court letters, plans of care, change of placement forms, SARs, referrals, progress notes, etc.).
- Maintain accurate, up‑to‑date information online for family demographics and team member contact details.
- Collaborate with other individuals involved with the youth and family.
- Report emergencies or critical incidents per Wraparound Policy #14.
- Attend in‑services, staff meetings, and consultations as needed.
- Assist with child and family coverage for coworkers when directed.
- Perform other job‑related duties as necessary to support the role.
- Bachelor’s degree in a relevant field of education or human services and a minimum of one year of continued experience providing mental health services.
- Complete 85 hours of mandatory training in Wraparound philosophy and policies, and attend a Family Orientation within 6 months of hire date.
- Working knowledge of computer applications (Windows, Microsoft Word).
- Demonstrated cultural competence and ability to respond respectfully to people of all cultures, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.
- Knowledge and skills to work with children, young adults, and families.
- Valid driver’s license, automobile, and insurance meeting agency requirements.
- Meet all employee requirements (references, criminal background check, drug test, driver’s license check).
Follow the agency Code of Conduct and established policies. Dress respectfully and maintain a professional demeanor. Do not post client‑identifying information on social networking sites. Conduct self ethically and maintain respectful relationships with all stakeholders. Show sensitivity toward cultural, ethnic, and disability issues and demonstrate commitment to agency values and mission.
Knowledge,Skills and Abilities
Working knowledge of positive child/youth development, patience and understanding of traumatic life situations, ability to interact calmly with escalating youth or parents, follow oral and written instructions, respond appropriately in crisis situations, proficiency with Microsoft Word and Excel, basic computer functions, efficient use of time, ability to multi‑task, and sensitivity to cultural and disability issues.
Work Relationship and ScopeReports directly to the Wraparound Supervisor. Has contact with clients and family members, program staff, consulting psychologists/psychiatrists, other collateral contacts, neighbors, funders, Milwaukee County departments, court officials, school staff and administrators, staff of youth‑serving agencies, and the general public.
