S.M.I.L.E. INC in Milwaukee, WI is looking for a compassionate and driven Case Manager to join our dedicated team. • Assess client needs and develop individualized service plans to support their goals
• Coordinate and connect clients with community resources, social services, and support programs
• Monitor client progress and adjust care plans as needed through regular follow-ups
• Maintain accurate and up-to-date case documentation and client records
• Collaborate with internal team members, external agencies, and service providers
• Advocate on behalf of clients to ensure access to appropriate services and support
• Conduct intake assessments and crisis intervention when necessary

• Bachelor's degree in Social Work, Human Services, Psychology, or a related field
• Prior experience in case management, social services, or a related role preferred
• Strong knowledge of community resources and social service systems in the Milwaukee area
• Excellent communication, organizational, and problem-solving skills
• Ability to manage a caseload effectively while maintaining attention to detail
• Empathetic, culturally competent, and client-focused approach to care
• Proficiency in case management software and Microsoft Office Suite
